For all patients at Eyam Surgery the named GP will be Dr Taylor. This is because Dr Taylor is the GP partner and so will be in a good position to take administrative responsibility for patient care.

You do not need to take any further action. We expect you to continue to see the GPs and nursing staff that you normally see. If you have any questions about this matter please speak to a member of the Practice Management Team (PMT), on 01433 630836 extension 2.

You can also speak to the GP you usually see the next time you have an appointment with them.
